How your health information is protected

Health information you provide in connection with care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA).

Who holds your medical record

Your care is provided by independent licensed healthcare providers. Those providers are the covered entities responsible for your medical record and maintain their own Notices of Privacy Practices, which are presented to you during intake and available in the patient portal.

Mortal Healthcare's role

Mortal Science LLC, the operator of Mortal Healthcare, handles protected health information as a business associate of the treating providers and operates under business associate agreements with the parties in the care chain. It uses and discloses protected health information only as permitted by HIPAA and those agreements: for treatment, payment, and healthcare operations, and as required by law.

Safeguards

All transactions and all pages handling customer, patient, or medical information are encrypted in transit using SSL/TLS. Access to protected health information is limited to personnel and service providers who need it to perform their roles, under confidentiality obligations.

Your rights

HIPAA gives you rights in your medical record, including access, amendment, and an accounting of certain disclosures. These rights are exercised through your treating provider. For help reaching your provider, contact support@mortalhealthcare.com.
